The Consular Section of U.S. Embassy San José is pleased to announce an upcoming Consular Outreach event in Perez Zeledon. Embassy staff will be available to accept adult passport renewals and to perform notarial services.

WHEN: Monday, April 25, 2016 – Passport Renewal Application Acceptance and Notarials from 12:30 to 2:30, Embassy Town Hall at 2:30.

Passport Renewals: To be eligible, you must present a 10-year passport that is valid or has expired less than 5 years ago. (No applications for lost or stolen passports, no applications for minors’ passports.) You will need to submit a Form DS-82 (Pdf – 91Kb) and a 2”-by-2” passport photo with a white background. Please complete and bring with you the Form DS-82, you can obtain a copy from the State Department website: travel.state.gov/content/passports/english/passports.html. The price is 60,000 colones. All payments must be in colones. Exact change is greatly appreciated. Do not sign the DS-82 until you have spoken with the consular officer at the outreach.
Notarial Services: The presence of the person requesting the notary and legal proof of identity is required. The price is 27,200 colones per seal. Maximum 3 documents per applicant. All payments must be in colones. Exact change is greatly appreciated.
Social Security Card replacement applications: Please complete the required forms and bring them to the outreach and we will accept the applications for you. The forms are available at https://cr.usembassy.gov/u-s-citizen-services/social-security/fbu-inquiry-form/.
